Solution

This event occurs when Multi-Factor Authentication (MFA) fails, typically due to an incorrect code entered for SMS, voice, email, or Time-based One-Time Password (TOTP) factors, or a system failure.

In this context, a system failure is an unexpected issue that occurs during the MFA process. The log type does not contain specific data to distinguish between an incorrect code and a system error.
